To complete the form, you'll need to:

1. Enter the dates that you'd like your transfer history for 
2. Provide your address, full name and phone number
3. Take or upload a picture of your ID
4. Take or upload a picture of you holding your ID
5. Accept the Terms and Conditions
6. Click Submit
